ACCESS AIDS Care is seeking a Full-Time HIV/AIDS Case Manager. Primary responsibilities will include HIV medical case management services, housing placement services, benefit coordination, etc. Successful candidate will provide comprehensive individualized services to individuals living with HIV/AIDS in the eastern region of Virginia. Preference is for individuals who have some service or work experience providing housing/homeless services.
Successful candidates are required to possess a minimum of a Bachelor’s Degree in Social Work/Human Services Counseling or related field plus 2 years experience in case management. HIV/AIDS knowledge preferred. Candidates must demonstrative excellent oral and written communication skills. Computer skills are required and prefer proficiency and knowledge of Microsoft Office products. To qualify, a candidate must have a valid Virginia driver’s license, satisfactory DMV record, and proof of automobile insurance.
Employee must be able to work collaboratively with a variety of staff within the organization. ACCESS AIDS Care values individuals with strong work ethic and commitment to the cause. Successful candidates must be able to demonstrate experience in the development of individualized assessments and services plans for individuals who are experiencing homelessness and HIV/AIDS. Must be able to work in a team environment and have excellent communication skills. Successful candidate must possess a keen analytic, organization and problem solving skills that support and enable sound decision making.
